The Fate of Periprosthetic Osteolysis Treated with Curettage and Bone Graft around Stable Cementless Total Hip Arthroplasty Implants

Abstract
Purpose: To evaluate the results of periprosthetic osteolysis treate using curettage and bone graft and without changing the stable cementless total hip arthroplasty components.

Materials and Methods: Twenty three patients were followed up for more than 2 years and are includeed in the analysis. The procedure was performed on the acetabular sides only in five cases, on the femoral sides only in eight and on both sides in ten. Autogenous iliac unicortical bone plate was used to reconstruct the cortical defect in 15 of the 18 cases of femoral lesions.

Results: The union of the graft occurred at an average 2.8 months. A resorption of the graft was found in two cases. New osteolysis occurred at the acetabular side in two cases and at the femoral side in five. The linear wear rate of the polyethylene inserts was significantly reduced after reoperation that was performed on the acetabular or femoral side only. It was not signficantly reduced when both sides were operated on.

Conclusion: Curettage and bone graft for the treatment of periprosthetic osteolysis without changing the stable cementless total hip arthroplasty comonents demonstreated satisfactory results in short term follow-up.
